1. OWNERSHIP: I certify that I am the legal owner of the animal(s) above. As such, I am both legally and financially responsible and assume all risks, dangers, and responsibility for injuries for myself and to the named animal(s). I (Owner) understand and agree that I (Owner) am solely responsible for their financial and legal implications while in the care of Camp K9 & K9 Cuts.
2. COMMUNICABLE DISEASE: A communicable disease is any illness that an animal can contract when they commune together. Some common examples include, but are not limited to, kennel cough, conjunctivitis (eye infection), papillomavirus (mouth warts). Kennel cough is one of the most common illnesses that dogs contract. There are variations in the strains of kennel cough and the current vaccines are not designed to cross-protect against all types. Even if your dog is vaccinated, Owner understand and takes all responsible for the risk of their animal(s) contracting this illness. I (Owner) understand that while Camp K9 & K9 Cuts takes all reasonable steps to avoid communicable diseases, there is still a risk of acquiring a communicable disease while staying at Camp K9 & K9 Cuts. In the event that my animal(s) contract(s) such a disease while staying, I (Owner) assume all the risks and agree to pay the costs of all treatments, exams, diagnostics, and medications. I (Owner) further agree to hold the owner and staff of Camp K9 & K9 Cuts harmless from expenses incurred for such treatment.
3. HONESTY POLICY: Owner is not enrolling any animal(s) that has undisclosed health/temperament issues that could potentially jeopardize the health/safety of the other animals or staff. Camp K9 & K9 Cuts employs a well trained staff, and is willing to work with the Owner to make accommodations that ensure safety/comfort for all. Owner agrees to disclose any relevant animal history to Camp K9 & K9 Cuts prior to check-in, and agrees to report any subsequent illness after appointment to reduce widespread infection.
4. FLEA/TICK FREE: All animals must be flea/tick free. Should an animal be observed to have fleas or ticks, Camp K9 & K9 Cuts will reach out to the emergency contact to remedy. Flea/Tick bath treatments are available through Camp K9 & K9 Cuts at a rate of $15.00+ depending on severity and size of the animal.
5. INGESTION OF FOREIGN OBJECT: Camp K9 & K9 Cuts takes all reasonable precautions and closely supervises each animal to avoid foreign objects, however, it is possible that an animal may consume a foreign object. Camp K9 & K9 Cuts will not pay for, reimburse, or be held liable for the ingestion of any foreign object. This includes, but is not limited to, bedding, rocks, shrubbery, and toys or toy pieces. Please inform Camp K9 & K9 Cuts of any chewing/destructive behaviors.
6. GROUP PLAY: If Owner consents to group play, assessments and acceptance into a play group is at the sole discretion of Camp K9 & K9 Cuts staff. Assessments are based on personality, socialization, and observed play behavior. We do not discriminate against any breed of dog. Once a dog is accepted into a play group, they are continually assessed for aggressive or undesirable behaviors. Camp K9 & K9 Cuts reserve the right to remove, either permanently or temporarily, any dog from a play group at any time, with or without prior notice.
7. INJURY: Owner understands and agrees that during normal animal play, Owner's animal(s) may sustain injuries. Animal play is monitored by Camp K9 & K9 Cuts's staff to best avoid injury, but scratches, punctures, torn ligaments, and other injuries occur despite the best supervision. Owner further understands and agrees that neither Camp K9 & K9 Cuts nor any of its staff will be liable for any illness, injury, pregnancy, death, and/or escape of Owner's animal(s) provided that reasonable care and precautions are followed. Owner understands the potential for injury to oneself while on the property of Camp K9 & K9 Cuts. Camp K9 & K9 Cuts is not held liable for any injuries from any animal(s) to owner(s) while on the property.
8. PERSONAL PROPERTY: Camp K9 & K9 Cuts will not be held responsible for lost or damaged property. To protect Owner's property and the safety of the animal(s), personal bedding will be removed if the animal is chewing/damaging it. Personal bedding must be washable. If it is not washable, we will replace it in the kennel. Bedding will be washed & dried if soiled and replacement bedding will be provided.
9. FOOD/MEDICATION: Owner agrees that it is the Owner's responsibility to leave an adequate supply of food (if not using kennel supply) and medications for Owner's animal(s) during the entire time Owner's animal(s) is cared for by Camp K9 & K9 Cuts. Should the food/medication supply need replacement, the Owner authorizes Camp K9 & K9 Cuts to provide alternate food and/or purchase replacements. In the event Camp K9 & K9 Cuts purchases a replacement, the Owner may be required to reimburse Camp K9 & K9 Cuts for the actual food/ medication costs and, in addition, up to $25.00 time/distance fee (per occurrence).
10. PHOTO AND VIDEO RELEASE: Owner agrees to allow Camp K9 & K9 Cuts to use Owner's animal(s) name and any images or likeness of Owner's animal(s) taken while he/she is at Camp K9 & K9 Cuts, in any form, for use at any time, in any media, marketing, advertising, illustration, trade or promotional materials with or without compensation, and Owner releases Camp K9 & K9 Cuts all rights that Owner may possess or claim to such image, likeness, recording, etc,.
11. PAYMENTS: Owner will be required to pay in full at the end of each visit for boarding, and at the end of each week for daycare. Payments can be made by cash or check. A $25.00 NSF charge will be added to your account for returned checks.
12. CLOSING TIME: {Hours Sunday - Saturday: 8-11am and 3:30pm-7:30pm} Owner agrees that if a animal(s) is/are not picked up by closing time, Owner herby authorizes overnight boarding appropriate for animal(s) and to pay Camp K9 & K9 Cuts for applicable overnight boarding charge, as well as any additional days due to holiday closures after no-show.
13. ABANDONMENT: If any animal(s) is left at Camp K9 & K9 Cuts for a minimum of 7 days, after the expected stay, without any contact from the owners, they will be considered abandoned. Any further measures will be taken to ensure the animal(s) are moved to an appropriate environment. Re-homing to a shelter may be necessary.
14. RIGHT OF REFUSAL: Camp K9 & K9 Cuts reserves the right to refuse admittance to any animal(s) or dismiss any animal(s) that do/does not meet or maintain the health, temperament or other daycare/boarding standards. Clients may also be refused for failure to settle previous billing statements. The determination shall be made at the sole discretion of Camp K9 & K9 Cuts.
15. FULL FORCE AND EFFECT: Owner further understands and expressly agrees that each and every of the foregoing provisions contained in paragraphs 1-15 shall be in force and effect and shall apply to each and every occasion on which Owner's animal(s) stay/s with Camp K9 & K9 Cuts for daycare, extended boarding, or other services, as the case may be. This Agreement shall remain in full force and effect, as between the parties, for all animal(s), including future animal(s) that join the Owner's family until and unless otherwise canceled or suspended by a writing signed by the parties.

I (Owner) hereby certify that I have read and understand this Waiver, Release of Liability, and the regulations set forth above. By signing this agreement, I (Owner) agree to be bound by its terms and conditions.
Cancelation Policy
We do not hold any cancelation fees but we kindly ask that you inform us if you no longer are in need of your reservation as soon as possible.
Our boarding facility has a limited amount of kennels and we are often booked out about 1-2 weeks in advance (much farther over holidays/school vacations). With our limited space, we frequently have to turn down customers looking to board on short notice, and have a waiting list for 1-2 weeks out. We ask that you contact us as soon as you know you will not be needing your reservation any longer, this way we can open up that kennel for others looking to stay with us. If you do not call to cancel your reservation, you are taking up a kennel from others waiting to get in. If you do not show up by end of day on the date you have scheduled with us, without notice of any reservation changes, we will cancel your boarding stay and open that kennel back up to the public the following day.
Due to our high demand for kennels and groom appointments, we hold a 3 strike policy. If we have 3 no-shows (failure to inform us you will no longer be needing a reservation or an appointment) we will need a down payment before we will reserve you for any future boarding or daycare reservations and/or grooming appointments.
Please call us to cancel a reservation if your dog will not be attending their daycare, boarding, or grooming reservation, thank you for your understanding!
